Patient Policies 

Rachel Hatch provides treatment for clients aged 6 and older for medication management as well as brief supportive therapy. Services are offered in the state of Colorado both in person and via telehealth. Please be aware that we DO NOT provide crisis management services. In case of an emergency or life-threatening situation, please dial 911 immediately or visit your nearest emergency department. 

Prescription Refills 

At each visit, please know which of your medications need to be filled. Prescriptions will primarily be completed during regular office visits, and last until the next scheduled visit. If you have to reschedule, please also let me know if you need a refill prior to the rescheduled visit. Similarly, if you miss a visit without calling to reschedule, it is your responsibility to notify me if you need a medication refill. Allow three business days for this refill to be completed. I can only provide one refill without attending an appointment. Patients must be seen at least every three months for me to provide medication refills. New medications will only be considered at an office visit, not over the phone.

APPOINTMENTS

CANCELLATIONS / LATE ARRIVAL POLICIES 

We kindly request a minium of  a 24-hour notice for apopintment cancellations or rescheduelings. Arriving 10 minutes late for a follow-up appointment or 15 minuteslate for an intake may lead to cancellation or rescheduling. Please see fee details below.

CANCELLATION/NO SHOW FEE

Patients who cancel an appointment with less than 24 -hour notice or fail to show for their appointment are subject to a $100/$150 fee.  Please be aware that you hold finacnial responsibility for appointments not rescheduled or canceled with appropriate advanced notice.  $100 cancellation/no show fee for established patients and $150 cancellation/ no show fee for new patients. 

EMERGENCY 

If an emergency necessitates a change to your scheduled appointment please reach out as soon as possible so that we can try to accommodate your needs. Immediate rescheduling of your appointment cannot be guaranteed

MISSED APPOINTMENTS

Patients with repeated missed appointments may be discharged from the practice. 

FMLA and DISABILITY 

  • Psychiatric Evaluations for Employment an Disability:  please note that we do not provide one-time psychiatric evaluations for proposes such as employment, medical clearance, disability claims, or legal matters.  However, for established clients, we will consider such evaluations on a case-by-case basis.  If you require our provider’s assistance in filling out STD/FMLA forms, please schedule an appointment to discuss this matter.

Patient- initiated telephone calls and/or email communications (greater than 15 minutes)

$75 fee

Short-tem disability forms and/or paperwork

$50- $100 fee
